    This plan is provided by the Unum Life Insurance Company of America and is administered by O'Neill Consulting Corporation.

    1/ This plan covers injuries or illnes that happen outside of work. If you are hurt on the job you will be covered by the Workers Compensation Policy of the employer.

    2/ Annual Premium of $368.00 is deducted from your Vacation Fund check.  (Employers contribute to the Vacation Fund.)

    3/ Waiting Period is 60 days

    4/ Benefit pays 60% of salary based on previous year's earnings

    5/ $5000 per month cap

    6/ Members 65 and older will only be covered for 18 months

    (After 1 year Unum will require that you to apply for Social Security Disability. You are still eligable to collect their plan, however they require that you begin the process of filing for SSDI.)

    7/ This Policy includes Group Life Insurance of $20,000 for death* and $10,000 for dismemberment                                          

    *Be sure to designate a beneficiary for your Life Insurance by using the UNUM Beneficiary Form Below. Make a copy for yourself and mail the original to O'Neill Consulting: Attn:  Pam Munnelly.

    NOTE:  During disability if you are receiving any other income, including unemployment compensation, or New Jersey TDB, you must disclose this on the claim form where indicated. If you receive LTD benefits from the Plan and do not disclose any income that you are receiving, your future benefits from the Vacation Fund, including Vacation Fund checks, will be forfeited until the amounts that have been overpaid to you due to this additional income, have been repaid in full back to the Vacation Fund.
